胸膜切除术加胸腔灌注化疗治疗恶性胸膜间皮瘤

Treatment of malignant pleural mesothelioma with pleurectomy and intraplueral infusion chemotherapy

目的:探讨恶性胸膜间皮瘤(malignant pleural mesothelioma,MPM)的有效治疗方法。方法:对23例行手术治疗的MPM患者在术后即行胸腔灌注化疗,随访分析患者的中位生存时间(median sunival time,MST)和1,2,3年生存率。结果:所有患者的MST为15个月(3～89个月),1、2和3年的生存率分别为69.6%、43.5%和26.0%。Ⅰ期、Ⅱ期以及Ⅲ期的1年生存率分别为83.3%、62.5%和33.3%,2年生存率分别为50%、37.5%和33.3%,3年生存率分别为34.0%、25.0%和0%。随访时间为2～3年,随访率达87.0%。结论:对于MPM患者采取以手术为主的综合治疗,可在一定程度上改善患者的预后及生存质量。

Objective ： To investigate an effective approach for malignant pleural mesothelioma （MPM）. Methods ： Twenty-three MPM patients were given intraplueral infusion chemotherapy immediately after surgery. Results：The median survival time was 15 months （3-89 months）. The 1-, 2-, and 3-year survival rates were 69.6%, 43.5%, and 26.0%, respectively. For patients at Ⅰ ,Ⅱ, andⅢ stage, the 1-year survival rates were 83.3% , 62.5% , and 33.3% , respectively; the 2-year survival rates were 50.0%, 37.5% , and 33.3%, respectively; the 3-year survival rates were 34.0%, 25.0%, and 0%, respectively. The follow-up rate was 87.0%. Conclusion ： Surgical treatment is an appropriate treatment for MPM patients. It improves the prognosis and life quality of MPM patients to a certain extent.
