摘要
目的探讨评估慢性阻塞性肺疾病(COPD)患者运动耐力的静态肺功能指标。方法对34例稳定期COPD患者进行常规肺通气功能、弥散功能、肺容量、最大吸气压(MIP)、呼气流速(EFL)受限检测和症状限制递增功率运动试验。结果第一秒用力呼气容积(FEV1)(r=0.493,P<0.05)、第一秒用力呼气容积占用力肺活量百分比(FEV1/FVC)(r=0.437,P<0.05)、最大分钟通气量(MVV)(r=0.583,P<0.05)、一氧化碳弥散量与肺容量比占预计值百分比(DLCO/VA%pred)(r=0.521,P<0.05)与最大运动功率(WRpeak)呈显著正相关关系,年龄(r=-0.472,P<0.05)和5分法EFL评分(r=-0.477,P<0.05)与最大运动功率呈显著负相关关系。15个变量中只有MVV对COPD患者的运动耐力有显著预测意义。结论MVV可作为评价COPD患者运动耐力的静态肺功能指标。
Objective To estimate exercise endurance in COPD patients using resting pulmonary function indexes. Methods Pulmonary ventilation function, diffusing function, lung capacity, MIP, EFL and symptom - limited progressive cycle ergometer exercise tests were performed in 34 stable COPD patients with Cosmed pulmonary function system. Results FEV1(r=0.493,P〈0.05), FEV1/FVC(%)(r=0. 437,P〈0. 05), MVV(r=0.583,P〈0.05), DLCO/ VA% pred ( r = 0. 521, P 〈 0. 05 ) were positively correlated with WRpeak. Age ( r = - 0. 472, P 〈0. 05 ) and 5 - point EFL score( r = -0. 477 ,P 〈 0. 05 ) were negatively correlated with WRpeak. Other variables have no significant correlation with WRpeak. In a multiple regression analysis, MVV was the only predictor of exercise endurance in 15 variables among COPD patients ( t = 3. 659, P 〈 0.01 ). Conclusion MVV can be used as a resting pulmonary function index to estimate exercise endurance in COPD patients.
