增殖细胞核抗原在胃肠道平滑肌肿瘤中表达的临床意义

Expression and significance of PCNA in gastrointestinal smooth muscle tumors

目的探讨增殖细胞核抗原(proliferative cell nuclear antigen,PCNA)表达与胃肠道平滑肌肿瘤(gas-trointestinal smooth muscle tumor,GISMT)的关系。方法采用免疫组化 S-P 法检测86例 GISMT 中 PCNA的表达情况。结果 PCNA 表达按正常胃肠道平滑肌组织、平滑肌瘤、平滑肌肉瘤Ⅰ、Ⅱ、Ⅲ级的顺序依次明显递增(P<0.01)。同时,PCNA 的表达与肿瘤的性质、大小、中心有无坏死亦有明显关系(P<0.01)。PCNA 低表达组的5年生存率明显高于高表达组(P<0.05)。结论 PCNA 是反映 GISMT 生物学特性的良好标志物,可作为判断 GISMT 性质、恶性程度、转移及预后的客观指标。

Objective In order to determine the relationship between proliferative cell nuclear antigen (PCNA) ex- pression and prognosis of gastrointestinal smooth muscle tumors(GISMT).Methods We studied 86 cases of GISMT by the immunohistochemical staining S-P method for PCNA.Results The expression of PCNA in- creased significantly according to the tollowing order:normal smooth muscle tissues of the gastrointestinal tract, leiomyomas,grade-Ⅰ leiomyosarcomas,grade-Ⅱ leiomyosareomas,gade-Ⅲ leiomyosarcomas(P<0.01);Mean- time,the expression of PCNA was associated with the tumors with or without contiguous organ invasion or distant metastasis,the size of tumors,the tumors with or without central necrosis(P<0.01).The five-year survival rate was significant higher in patients with low-expression of PCNA than those with high-expression of PCNA(P< 0.05).Conclusion PCNA is the valuable indicator of biological characteristics of GISMT,and can be used as an objective parameter in distingushing malignants from benigus,judging the malignant degree and predicting the prognosis of the patients with GISMT.
