摘要
目的调查并分析我国全科主治医师职称晋升前后的情况及发展方向,为制定相关政策提供参考依据。方法2007年11月至2008年3月,对天津市、南京市和武汉市2001年至2007年间通过全国全科医学专业中级技术资格考试的主治医师524人进行问卷调查,并进行专题访谈。结果524人通过晋升考试,其中注册为全科医学专业者仅133人(占25.4%);晋升后,工作在临床科室岗位的减少52人,其中21人转入管理岗位;工作在一级医院、卫生院、社区卫生服务机构和企事业单位医院、保健站的人数减少了40人,全民医疗机构流失人数多达11人。结论目前我国全科医学专业执业注册人员数量少;全科主治医师队伍不稳定,流失严重。
Objective To study current status of health professionals before and after promotion to general practitioners (GPs)-in-charge to provide evidence for related policy decision-making. Methods From November 2007 to March 2008, 524 general practitioners-in-charge who passed national qualification examinations of medium-level health professionals were interviewed with questionnaire, as well as by focus group discussion, in cities of Tianjin, Nanjing and Wuhan during 2001 to 2007. Results One hundred and thirty-three of 524 respondents (25.4%) registered at local health bureaux as GPs to practise medicine, and 52 left their original clinical positions and 21 of 52 moved to administration positions. Forty of them left primary-care hospitals, township hospitals, community health-care service centers or hospitals and clinics affiliated to enterprises, where they originally worked with. Eleven of them left state-owned institutions. Conclusion At present, the number of GPs registered for permit to practice medicine decreased and workforce of GPs drained out seriously appearing in unstable status.
