86例丘脑出血的临床分析

Clinical Analysis of 86 Cases of Thalamic Hemorrhage

目的探讨丘脑出血的临床特点、诊断和治疗。方法回顾性分析我院1998年1月-2008年1月所诊治的86例丘脑出血的临床资料。结果本组86例中,手术治疗39例,非手术治疗47例,死亡43例,放弃治疗8例,存活35例,存活病员ADL分级:Ⅰ级18例,Ⅱ级2例,Ⅲ级10例,Ⅳ级3例,Ⅴ级2例。结论丘脑出血病情危重,变化快,其预后与出血部位及出血量有关,应采取个体化治疗措施,选择适当的治疗方法,后期加强护理,注重防治并发症,能提高治愈率、改善预后。

Objective To investigate the clinical characteristics, diagnosis and treatment of thalamic hemorrhage. Methods The clinical data of 86 cases with thalamic hemorrhage from Jan, 1998 to Jan 2008 were analyzed retrospectively. Results Among 86 patients,39 were with surgical treatment ,47 with other pathway ;43 died, 8 abandoned, 35 Survived ; the ADL prognosis scale：18 cases were grade Ⅰ ,2 cases grade Ⅱ ,10 cases grade Ⅲ ,3 cases grade Ⅳ,and 2 cases grade Ⅴ. Conclusion Thalamic hemorrhage is serious and changeable. The prognosis is close relatively to the hematoma volume and location. The individualized measures, proper therapeutic method, intensifying the nursing care in rehabilitation time and avoiding complications can effectively increase the curative ratio and improve the prognosis.
