早期应用前列腺素E_1对大鼠烟雾吸入性肺损伤的保护作用 被引量:1

RESEARCH ON PROTECTIVE EFFECTS OF PGE_1 IN RATS WITH SMOKE INHALATION PULMONARY INJURY

摘要 目的:探讨早期应用前列腺素E1(PGE1)对烟雾吸入性损伤大鼠的肺保护作用及机制。方法:54只大鼠随机分为3组:正常组、损伤组和PGE1组,建立大鼠烟雾吸入性损伤模型。PGE1组于烟雾吸入伤后30 min即予PGE1雾化吸入治疗,每隔8 h雾化治疗1次。观测损伤组和PGE1组大鼠伤后6、24、48 h肺脏病理、动脉血气分析、支气管肺泡灌洗液(BALF)总蛋白水平(TP)、肺组织湿干重比(W/D)及一氧化氮(NO)、总一氧化氮合酶(总NOS)、诱导型一氧化氮合酶(iNOS)、结构型一氧化氮合酶(cNOS)等指标的变化。结果:PGE1治疗后肺弥漫性充血、出血、水肿、大量中性粒细胞(PMN)浸润等改变范围及程度较损伤组均有所减轻;PaO2水平与氧合指数(PaO2/FiO2)明显提高;支气管肺泡灌洗液TP水平、W/D值较损伤组明显减低;在24、48 h,PGE1组的肺组织NO与总NOS水平较损伤组有下降趋势,而iNOS水平在48 h则明显低于损伤组。结论:早期雾化吸入PGE1可以改善烟雾吸入导致的大鼠肺水肿、出血等急性肺损伤,其保护作用可能是通过降低肺部毛细血管通透性、抑制iN-OS活化等途径实现的。 Objective: To evaluate the protective effects of PGE1 and its basic mechanism in rats with smoke inhalation pulmonary injury. Methods: 54 rats were randomly divided into three groups: control group, injury groups and PGE1 groups. In PGE1 groups, aerosolized PGE1 were inhaled every 8 hours one time within the 0.5-48 hours after smoke inhalation injury. In injury groups and PGE1 groups, the indexes which we detected at the 6,24 and 48 hours after injury were as follows: analysis of pathology in the lung, blood gas analysis, ratio of wet weight/dry weight(W/D) of the lung tissue, the detections of NO, total NOS,iNOS, cNOS in the lung tissue, total protein (TP) in the BALF. Result. Diffuse congestion, hemorrhage, edema, infiltration of PMN occurred in the lung tissue after smoke inhalation injury, the extent and degree of these lesion were decreased after the treatment of inhalation of aerosolized PGE1. In PGE1 groups, the levels of PaO2 and PaO2/FiO2 were significantly higher than that in injury groups while TP and W/D levels were markedly lower than that in the injury groups. In injury groups and PGE1 groups, there was a decreasing tendency of the NO and total NOS in PGE1 groups, especially, the levels of iNOS were lower than injury groups. Conclusion: Inhalation of aerosolized PGE1 can alleviate the smoke inhalation pulmonary injury through depressing vascular permeability in the lung and depressing the expression of iNOS.
