摘要
目的:探讨组织多肽特异性抗原(tissue polypeptide specific antigen,TPS)、细胞角蛋白19片段(cytokeratin 19 fragment,CYFRA21-1)和癌胚抗原(CEA)在肺鳞癌及腺癌患者血清中的表达水平及其对肺癌诊断的临床应用价值。方法:采用酶联免疫吸附法(ELISA)和免疫放射分析法(IRMA)对60例肺鳞癌及腺癌患者、20例肺部良性疾病患者的血清TPS,CYFRA21-1和CEA进行检测。结果:肺癌组血清TPS、CEA和CYFRA21-1水平均明显高于肺部良性疾病组(P<0.05)。TPS,CYFRA21-1和CEA的水平均随着临床分期的进展而升高。TPS、CEA和CYFRA21-1三者联合检测的敏感性为90.0%,较各单组检测敏感性明显上升(P<0.05)。结论:TPS、CEA和CYFRA21-1联合检测肺癌有重要意义。
Objective: To study the expression of tissue polypeptide specific antigen (TPS), careinoembryonic antigen (CEA) , cytokeratin 19 fragment( CYFRA21 - 1 )and their clinical diagnostic evaluation in patients with lung carcinoma. Methods: The serum levels and positive rate of TPS, CYFRA21 -1 and CEA were determined by enzyme -linked immunosorbent assay (ELISA) and immunoradiometric assay (IRMA) in 60 patients with lung cancer, and 20 patients with benign diseases. Results: The serum levels of TPS , CEA and CYFRA21 - 1 in lung cancer group were significantly higher than those in the normal control group ( P 〈 0.05 ). And the levels and positive rate of TPS, CYFRA21 - 1 and CEA became even higher as the malignancy progressed. The joint detection of TPS,CEA and CYFRA21 -1 had the highest sensitivity (90.0%) (P 〈 0.05). Conclusions: The joint detection of TPS,CEA and CYFRA21 -1 has an important role in diagnosis of lung cancer.
