内镜治疗直肠大息肉的临床价值

Clinical value of endoscopic resection of large rectal polyps

目的探讨内镜下完整切除直肠大息肉的临床价值。方法对2005年1月～2007年12月进行内镜切除的59例直肠大息肉患者的临床资料进行回顾性分析。结果8例(12.9%)出现明显的出血,所有的出血通过内镜下治疗均得到有效的控制,仅有1例病人出现了穿孔(1.6%)。腺瘤性息肉是该组中最常见的病变(85.5%),4例出现恶变。随访6～36个月,4例腺瘤病人复发,再次在内镜下完全切除。结论内镜下切除直肠大息肉是一项可行的、安全有效的技术。

【Objective】 To evaluate the clinical value of endoscopic resection of large rectal polyps. 【Methods】 The clinical data of 59 cases of large rectal polyps treated with endoscopic resection from January 2005 to December 2007 was analysed retrospectively. 【Results】 Bleeding occurred in 8 patients after polypectomy （12.9%）. All bleedings were treated endoscopically. Perforation occurred in 1 patients （1.6%）. Histology results showed that adenoma was the most frenquent lesion （85.5%） and coexistent malignancy in 4 cases. During a meaning follow-up period of 16 months, 4 patients with benign polyps developing recurrences were treated endoscopically. 【Conclusions】 Endoscopic resection of large rectal polyps is a safe and effective technology.
