摘要
目的:探讨血浆脑钠肽(BNP)值结合超声心动图(UCG)相关参数对心脏病患者心功能的评估价值。方法:测定147例住院心脏病患者血浆BNP值,同时测定UCG各相关参数。分析患者静息心率、心功能分级、左室射血分数(LVEF)、二尖瓣口舒张早期与舒张晚期血流峰值速度比值(E/A值)、左室收缩末径(LVEDs)、左室舒张末径(LVEDd)、左房内径(LAD)、左室短轴缩短率(LVFS)等因素与BNP的相关性。结果:血浆BNP水平随NYHA心功能分级程度的升高而显著增加(r=0.645,P<0.001)。BNP与LVEF呈负相关(r=-0.451,P<0.001)。BNP与LVEDs、LVEDd的Pearson系数分别为0.473、0.401(P<0.01)。结论:BNP水平与NYHA心功能分级高度相关,可预测LVEF的下降,能较好地反映心脏病患者左室功能状态;BNP结合UCG相关参数,是一种无创、简便、敏感的评价心功能的方法。
Objective:The aim of this study was to evaluate the heart function by plasma brain natriuretic peptide (BNP) and ultrasoundcardiography (UCG), and further to evaluate the clinical value of BNP in diagnosis and illness evaluation of heart failure patients. Metbod: We measured the plasma BNP levels and UCG parameters of 147 patients with heart disease. Correlation analysis was made between plasma concentrations of BNP and LVEF, E/A, Hr, MAP, NYHA classification, LVEDs, LVEDd, LAD. Result: The plasma BNP level correlated positively with NYHA class (r= 0. 645,P〈0. 001)and negatively with LVEF(r= -0. 451, P〈0. 001). Pearson coefficients are 0. 473, 0. 401 between BNP and LVEDs, BNP and LVEDd, respectively (P〈0. 001). Conclusion.. The concentration of plasma BNP correlated with NYHA classification and the degree of LV systolic and diastolic dysfunctions evaluated by UCG. BNP measurement combined with UCG offer useful information regarding heart failure.
