不同营养支持对住院患者营养状况影响的对比观察

Comparative study of the effect of different nutritional support route on nutrition of the in-patients

目的:探究肠内营养(NE)、肠外营养(PE)、肠内肠外营养(NE+PE)的临床效果,为营养支持的选择提供参考。方法:对北京大学深圳医院2007年1月-2008年2月期间ICU病房、胃肠外科、消化内科、脑外科等13个科室的住院患者的营养状况进行回顾性调查,按NE、PE、NE+PE进行分组,从3组中分别随机抽取51,54,51例患者,考究其营养支持前后血红蛋白(Hb)、血清白蛋白(sALB)的变化以及住院天数,分别进行比较分析。结果:3组患者的Hb及sALB均有不同程度的降低,组间差异均存在显著性(P<0.05),NE+PE能显著减少Hb及sALB的降低。NE+PE与NE的住院天数差异无显著性(P>0.05),较PE则显著减少患者的住院时间(P<0.05)。结论:3组相互比较,NE+PE在减少住院患者Hb及sALB降低的效果显著,且较PE减少患者的住院天数有优势,是一种值得推广和完善的营养支持方式。

OBJECTIVE To investigate the clinical effect of enteral nutrition （NE）, parenteral nutrition （PE） and enteral parenteral nutrition （NE ＋ PE）, and provide referenee to clinical practise. METHODS Retrospetive study of nutrition of inpatient was conducted in ICU ward, gastrointestinal surgery, digestive medicine, brain surgery and other sections of the 13 wards of Peking University Shenzhen Hospital in January 2007 to February 2008, Patients were divided into NE, PE or NE ＋ PE group according the nutritional route, and each group was 51, 54 and 51 cases espectively. Hemoglobin （Hb）, serum albumin （sALB）, as well as the changes in the number of days hospitalized were the index for comparison. RESULTS The level of Hb and sALB were descend in three groups and there was significant difference of descend content among the three group（P（ 0. 05）, NE ＋ PE could significantly reduced the descend of Hb and sALB, compared to PE, NE and NE ＋ PE reduce the hospitalization days significantly （P（0. 05）, no obviously difference between NE and NE＋ PE（P〉0. 05）. CONCLUSION NE ＋ PE could reduce the descend of Hb and sALB significantly and shorten hospitalized days than PE.
