胃肠道癌卵巢转移66例临床分析

Clinical analysis of 66 metastatic ovarian tumors from the stomach or colon cancer

目的:分析胃肠道癌卵巢转移癌的临床特点、治疗及预后因素。方法:对66例胃肠道癌卵巢转移患者的临床特征、治疗、生存情况及预后因素进行回顾性分析。结果:转移性卵巢癌的病理类型以低分化腺癌及印戒细胞癌为主(占62.12%,41/66),双侧卵巢转移常见(65.15%,43/66);来源于胃癌者比来源于大肠癌者且预后差(P=0.016);全组患者中位生存期为10个月,生存率分别是:1年3 4.3%,2年1 9.1%,3年8.9%。卵巢转移癌经过治疗者较放弃治疗者生存期延长(P=0.000)。术后残留肿瘤≤2 cm者较>2 cm者生存时间延长(P=0.002)。结论:胃肠道癌卵巢转移病期晚,预后差,以手术联合化疗为主的综合治疗有延长患者生存期的趋势。

Objective：To investigate the clinical features, treatment and prognosis of metastatic ovarian carcinoma. Methods ：The clinical data of 66 patients with metastatic ovarian carcinoma at was reviewed. Results： The primary carcinoma included 32 cases of stomach cancer and 34 cases of colon cancer. Ovarian metastasis was found bilateral （65.15% ,43/66 ）, especially in low differentiated or ring - cell adenocarcinoma, when the primary tumors were at stomach, the prognosis was poorer （P = 0. 016）. In 66 patients, the median survival was 10 months . Survival rates of 1 year,2 years and 3 year were 34.3% , 19.1% ,8.9%, respectively. In the patients who underwent operation, if the diameter ≤2cm,the survival time prolonged, compared with 〉 2cm （P = 0. 002）. Conclusion： Ovarian metastatic tumors from stomach and colon have poor prognosis. Operation and chemotherapy may improve the survival.
