COPD气管切开患者的拔管时机研究

The research of extubation time for tracheotomy patients with COPD

目的研究COPD气管切开患者的拔管时机。方法13例行机械通气治疗的COPD气管切开患者随机分为两组,试验组6例,当自主呼吸试验时间大于24h拔管,对照组7例,当自主呼吸试验时间大于2h拔管,比较拔管后48h内的再插管率。结果试验组和对照组的再插管率分别为16.7%和85.7%,差异有统计学意义(P<0.05)。结论COPD气管切开患者,当自主呼吸试验时间大于24小时的时候拔管具有较低的再插管率。

Objective To study the extubation time for traeheotomy patients with COPD. Methods 13 traeheotomy patients with COPD were divided two groups randomly, the experiment group had 6 cases, which proceeded extubation when self respiratory trial time was more than 24 hours, the controls had 7 cases, extubation when self respiratory trial time more than 2 hours. The re-intubation rates from groups were compared after extubated in 48 hours. Results The re-intubation rates from the experimental group and controls was 16. 7% and 85.7% respectively, the difference had statistic significance （ P 〈 0.05 ）. Conclusion For the tracheotomy patients with COPD extubation, there are lower re-intubatin rates when self respiratory trial time more than 24 hours.
