The prognostic value of serum hepatocyte growth factor in patients with unstable angina pectoris

摘要 目的:探讨血清肝细胞生长因子(HGF)对不稳定心绞痛患者预后的判断价值。方法:应用酶联免疫吸附法对56例不稳定心绞痛(UAP)患者、20例健康人行血清HGF、肌钙蛋白I(cTnI)、肌酸激酶(CK)、肌酸激酶同工酶(CK-MB)水平测定。根据HGF水平又将UAP分为2组:A组(HGF≤360ng/L),20例;B组(HGF>360ng/L),36例。观察住院期间2组UAP患者心脏事件发生率的差异。结果:A组血清HGF水平[(247.53±90.59)ng/L]明显低于B组[(571.03±160.23)ng/L,P<0.05];2组间血清cTnI、CK和CK-MB水平差异无统计学意义。UAP患者中,A组30d内发生急性心肌梗死、心源性猝死、顽固性心绞痛者明显高于B组(P<0.05);对上述心脏事件,HGF定量检测的敏感性为76.5%,阴性预测值为88.9%,准确性为80.4%。结论:HGF对UAP患者的预后有较好的判断价值,可作为预测UAP患者预后的一个指标。 Aim: To explore the prognostic value of serum hepatocyte growth factor (HGF) in patients with unstable angina pectoris (UAP) . Methods: Serum HGF, troponin I (cTnI) , creatine kinase (CK) and creatine kinase-MB ( CK- MB) were simultaneously measured in 20 healthy subjects and 56 patients with enzyme linked immune absorption assay (ELISA). 56 patients with UAP were divided into two groups by HGF level: group A (HGF≤360 ng/L) ,20 subjects and group B (HGF 〉 360 ng/L) ,36 subjects. To observe the incidence of cardiac events in two groups. Results: Serum HG-F levels in group A[(247.53 ±90.59)ng/L] was significantly lower than in group B[(571.03 ± 160.23)ng/L] (P 〈 0.05). Serum cTnI, CK and CK-MB had no statistical significance in group A and group B. Among 56 UAP patients, the incidence of cardiac events, including acute myocardial infarction(AMI) , sudden cardiac death and refractory angina pectoris in 30 days were significantly higher in group A than in group B (65.0% vs 11.1% ,P 〈 0.05 ). For cardiac events, the sensitivity was 76.5% , negative predictive value was 88.9% by serum HGF level. Conclusion: The study indicates that serum HGF level has a predictive value for prognosis of patients with UAP and can be used as a marker for prognosis of patients with UAP.
