成年癫痫患者生活质量及相关因素

Clinical Study on Quality of Life and Related Factors in Adult Patients with Epilepsy

目的:探讨我国北方地区成年癫痫患者的生活质量及相关因素。方法:在医院神经科门诊采用连续取样,共选择293例来自我国北方地区经临床确诊的成年癫痫患者,同时选择同期门诊常规体检的131例健康的成年人为对照组,对两组样本进行癫痫患者生活质量问卷-89评估,内容涉及其生活质量各个方面及影响生活质量的因素。结果:除药物影响和对发作的担忧外,成年癫痫人群生活质量总分及生活质量的15个方面得分均低于正常对照组[如,对健康的认识:(64.9±22.2)vs.(91.8±7.2),P<0.001]。癫痫人群的生活质量各方面中以药物影响和对发作担忧得分最低[分别为(46.4±17.1)和(51.8±13.1)]。生活质量总分与多种因素相关,未婚与已婚对比分析显示,已婚患者生活质量总分低于未婚患者[(57.1±15.3)vs.(70.9±15.7),P<0.001]。相关分析显示,生活质量总分与年龄、病程、发作严重程度、药物副作用、发作类型得分呈负相关(r=-0.27、-0.29、-0.43、-0.26、-0.16,均P<0.01),与受教育程度、经济状况和发作频率得分呈正相关(r=0.23、0.34、0.31,均P<0.01),分析进入回归方程的因素为年龄(β=-0.379)、病程(β=-0.408)、发作严重程度(β=-9.049)、教育程度(β=3.187)、经济状况(β=5.796)。结论:成年癫痫患者生活质量明显低于普通人群,患者的年龄越大、病程越长、发作严重程度越重,生活质量越差;而教育程度高、经济状况好的患者生活质量较高。

Objective： To study the quality of life （QOL） in adult patients with epilepsy in northern China. Methods： A total of 293 adult out-patients with epilepsy were selected with strict procedure of diagnosis by using continuous sampling method, and 131 healthy adult were adopted as a control group, and then the QOL in all subjects were evaluated with the Quality of Life in Epilepsy Inventory-89 （QOL -89, Chinese version） . The contents of evaluation were involved in many aspects of quality of life and the factors influencing quality of life. Results： Compared with control group, the total score and sub-scores of QOL （ except for effects of medication and worry about seizure ） for epilepsy patients decreased [ e. g. health perceptions （64. 9 ± 22.2 ） vs. （ 91.8 ± 7.2 ）, P 〈 0. 001 ] . The scores of worry about seizure （46. 4 ± 17. 1 ） and effects of medication （51. g ± 13.1 ） were lowest in patients with epilepsy. QOL total score of married patients was lower than that of unmarried patients [ （57. 1 ± 15. 3） vs. （70. 9 ± 15.7）, P 〈0. 001 ] . Correlation analysis showed that QOL total score was negatively correlated with age, course of disease, sever of seizure, antiepileptic drug and type of seizure （ r = -0. 27, - 0. 29, - 0. 43, - 0. 26, and - 0. 16 respectively, P 〈 0.01 ）, and was positively correlated with education background, economic status, and seizure epsode frequency （ r = 0. 23, 0. 34, and 0. 31 respectively, P 〈 0. 01 ） . Multi regression analysis showed that five factors came into regression equation including age （ β = -0. 379 ）, course of disease （ β = - 0. 408 ）, sever of seizure （ β = -9. 049） , education background （ β =3. 187） and economic status （ β=5.796） . Conclusion： Quality of life of adult patients with epilepsy is obviously lower than normal people and negatively related to age, course of disease and sever of seizure, while positively related to education background and economic status.
