新生儿不同程度窒息肾血流及肾功能的变化 被引量:2

摘要 目的探讨新生儿窒息后肾血流动力学及肾功能指标改变。方法测定71例窒息新生儿肾动脉的血流动力学参数、尿酶、血尿素氮、肌酐及尿常规,并与20例正常足月新生儿进行对比研究。结果新生儿窒息后肾血液灌流阻力增大,血液灌流量减少,尤以舒张期为甚。轻度窒息组即可出现明显的尿酶和血流动力学改变。结论肾血流动力学紊乱,阻力增大,血流量尤其舒张期血流量减少是新生儿窒息肾损伤的主要原因。运用多普勒超声观察其血流动力学变化和检测尿酶的变化可用于新生儿窒息肾损伤的早期诊断。 Objective To explore the changes of renal blood flow and renal injury in neonates with birth asphyxia. Methods Seventy-one patients were tested for Ultrasonic Doppler renal blood flow and renal function. Results The blood perfusion resistance was increased and the blood flow perfusion was decreased, Even in light asphyxia. Conclusion The hemodynamic disturbance is the main reason for renal injury and dysfunction in infants with birth asphyxia, and detecting hemodynamic disturbance and urinary enzyme may be an early diagnostic method to evaluate the renal injury.
