平阳霉素联合角膜缘干细胞移植术治疗翼状胬肉55例

Application of pingyangmycin combined with stem cell transplantation of limbus cornea in 55 cases with pterygium

目的探讨平阳霉素术前应用联合角膜缘干细胞移植术治疗翼状胬肉的疗效。方法对55例(61眼)翼状胬肉患者分别于术前采用平阳霉素8mg,地塞米松5mg,加生理盐水4mL稀释的溶液注射到胬肉颈、体部表面下1～2次,注射后6～10d再进行角膜缘干细胞移植术,术后随访8个月至3a。结果角膜上皮于术后1～2d自移植片向角膜创面生长;3～4d见创面角膜上皮光滑、愈合、完全透明。术后复发2眼,复发率为3.22%,余无其他并发症。结论平阳霉素术前应用联合角膜缘干细胞移植术治疗翼状胬内疗效显著,能明显降低术后复发率,值得推广。

Objective To investigate the curative effect of preoperative application of pingyangmycin combined with stem cell transplantation of limbus cornea for the treatment of pterygium. Methods A total of 55 cases （61 eyes） with pterygium were treated with preoperative injection of a mixed solution,which comprised 8 mg pingyangmycin, 5 mg methylprednisolone and 4 mL normal sodium, to pterygium body and undersurface for 1 time to 2 times, respectively, combined with stem cell transplantation of limbus cornea. All cases were followed up for 8 months to 3 years. Results Cornea epithelium grew from corneal graft to corneal wound in 1-2 days after operation, corneal would became smooth, healing and absolutely cleared in 3-4 days postoperative. Two eyes （3.22%） recurred with pterygium, and there was no other complications.Conclusion Preoperative application of pingyangmycin combined with stem cell transplantation of limbus cornea is an effective method to treat pterygium, and it can significantly reduce the recurrent rate.
