术后早期炎性肠梗阻67例诊治分析

Diagnosis and treatment of the early postoperative inflammatory bowel obstruction in 67 cases

目的探讨术后早期炎性肠梗阻（EPISBO）的诊断和治疗方法。方法回顾性分析2000年1月至2007年10月收治的67例EPISBO的临床资料。结果有64例经非手术治疗治愈；3例经非手术治疗5～7d后中转手术，2例治愈，1例发生肠瘘后死亡。结论术后早期炎性肠梗阻好发于腹腔污染重或创伤大的腹部手术，多发生于术后3周以内，有典型的肠梗阻症状和体征，诊断主要依靠病史、体征及腹部X线、CT检查，应先采取非手术治疗，要严格掌握再手术适应证。

Objective To explore the diagnosis and treatment of the early postoperative inflammatory bowel obstruction （EPISBO）. Methods Clinical data of 67 cases with EPISBO from January 2000 to October 2007 was retrospectively studied. Results Among the patients,64 cases cured with non-surgical treatment, 3 with the non-surgical treatment for 5 - 7 d treated with surgery,2 patients were cured and 1 death. Conclusion Early postoperative inflammatory intestinal obstruction occurred in heavy pollution of abdominal trauma or major abdominal surgery, and occur in less than 2 weeks after the operation. There are typical clinical symptoms and signs of intestinal obstruction. The diagnosis mainly depends on history, signs and abdominal X-ray, CT examination. It is best to get the non-surgical treatment firstly, and proper choose the surgical indication.
