摘要
目的探讨血管紧张素Ⅱ2型受体(AT2R)与血压水平及相关炎性因子的关系。方法采用横断面研究方法,从体检人群中随机调查138例为研究对象,根据美国预防、检测、评估与治疗高血压全国联合委员会第7次报告(JNC7)将入选对象分为3组,正常血压组31例(≤120/80mmHg)、高血压前期组39例(120~139/80~89mmHg)、高血压组68例(≥140/90mmHg);用酶联免疫吸附技术(ELISA)检测血浆AT2R水平,用放射免疫法检测肿瘤坏死因子α(TNF-α)、内皮素1(ET-1)及血管紧张素Ⅱ(AngⅡ)的水平,用自动生化检测仪检测血脂、血糖、肾功能。结果血浆AT2R在正常血压组、高血压前期组及高血压组间差异有统计学意义(P<0.01),且与收缩压(r=0.881)、舒张压(r=0.752)、平均动脉压(r=0.847)及脉压水平(r=0.587)呈正相关(P均<0.01);2)血浆AT2R与TNF-α(r=0.804)、ET-1(r=0.769)及AngⅡ(r=0.806)呈正相关(P均<0.01);3)多元线性逐步回归分析显示,年龄、AngⅡ及AT2R是平均动脉压的独立影响因素;AngⅡ及AT2R是收缩压的独立影响因素;ET-1及AT2R是舒张压的独立影响因素。结论血浆AT2R水平与血压水平及炎性因子水平呈正相关,是平均动脉压、收缩压及舒张压的独立危险因素。
Objective To investigate the relationship between plasma angiotension Ⅱ Type 2 Receptor (AT2 R), blood pressure and inflammatory factors. Methods One hundred and thirty-eight subjects were enrolled and categorized into three groups according to JNC 7 criteria; normal blood pressure(≤120/80 mmHg, n= 31), prehypertension(120--139/80- 89 mmHg, n 39)and hypertension(≥140/90 mmHg, n=68). Plasm AT2R were detected by ELISA, tumor necrosis factor-α(TNF-α), endothelin-1 (ET-1) and angiotension Ⅱ (Ang Ⅱ ) by radioimmunity. Results 1)Plasma AT2R increased with the increasement of blood pressure, and the intergroup differences were significant (all P〈0.01) ; Pearson correlation analysis revealed that plasma AT2 R was positively correlated with systolic blood pressure(r=0. 881), diastolic blood pressure(r=0. 752), mean arterialpressure(r=0.847) and pulse pressure(r=0.587, all P〈0.01); 2)Plasma AT2R was positively correlated with inflammation factors; TNF-α(r=0.804), ET-1(r=0.769) and Ang Ⅱ (r=0.806, all P〈0.01); 3)Multi variables regression analysis showed that age, Ang Ⅱ and AT2 R were independent predisposed factors for mean arterial pressure; Ang Ⅱ and AT2R were independent predisposed factors for systolic blood pressure; ET-1 and AT2R were independent predisposed factors for diastolic blood pressure. Conclusion Plasma AT2R was closely and positively correlated with blood pressure and inflammatory factors, which is independent predisposed factor for mean arterial pressure, systolic blood pressure and diastolic blood pressure.
