双镜联合胃楔形切除术微创根治胃Dieulafoy病20例报告

Gastroscopic-Assisted Laparoscopic Wedge Resection of the Stomach for Dieulafoy Disease

目的探讨双镜联合微创根治胃Dieulafoy病的优势。方法1999～2007年我院共收治胃Dieulafoy病20例,在胃镜引导下,腹腔镜下应用腔内切割吻合器(Endo-GIA)行胃楔形切除术。结果20例手术均获成功,无术后并发症,早期活动拔除胃管并进食。20例随访3～36个月,无再次出血。结论胃镜是胃Dieulafoy病的首选诊断方法,胃镜引导下腹腔镜胃楔形切除术应是此病的首选治疗方法,降低再次出血率和胃壁溃疡发生率。

Objective To evaluate the advantages of gastroscopic-assisted laparoscopic wedge resection of the stomach for the treatment of Dieulafoy＇ s disease. Methods From 1999 to 2007, 20 patients with gastric Dieulafoy＇ s disease were cured in our hospital by gastroscopic-assisted laparoseopic wedge resection of the stomach. Results The procedure was completed in all of the cases without postoperative complications. The gastrointestinal decompression tube was removed and activity was restored in an early stage in all the patients. They were followed up for 3 to 36 months, during which no one had gastric hemorrhage. Conclusions Gastroscope plus gastroscopic-assisted laparoscopic wedge resection are the first choice for the diagnosis and treatment of Diculafoy＇ s disease. The incidence rates of re-bleeding and gastric ulcer can be decreased by using the procedure.
