摘要
目的探索放射性支架研制、临床使用的可行性、安全性及初步疗效。方法选择合适的覆膜支架,于支架外周按一定的规律固定上特制的可容纳放射粒子的鞘。根据患者病变情况计算出放射粒子的剂量及排布规律,将放射性碘125粒子装进上述粒子仓内,制成放射性支架。选择8例食管和贲门癌患者,临床分级为Ⅱb ̄Ⅳ期,将放射性支架置入到患者病变部位并进行临床随访。结果8例患者放射性支架均释放到位,释放过程顺利,释放过程中未出现放射粒子脱落现象。术后1、2、3个月复查,吞咽困难程度持续保持在0级,未见局部肿瘤增长和新增转移灶,无一例出现局部严重疼痛、瘘、免疫功能受损、骨髓抑制等严重副作用。结论放射性支架在食管、贲门癌中的使用是安全、可行、有效的,取得良好的近期效果。
Objective To investigate the feasibility, safety, primary clinical effectiveness of irradiation stents. Methods The proper stent was first chosen, and the specially made sheath involving radiated particles was fixed outside the stent according to some rule. Based on the size of the tumor focus conditions,the radiated particles' dose and arrangement were computed, and put the 1-125 particles into the sheath, then the irradiation stent was finished. The irradiation stents were placed in 8 patients with esophageal and cardiac cancer of Ⅱb - Ⅳ grades,and the clinical effectiveness was observed. Results The irradiation stents were put into the sites of 8 diseased esophageal and cardiac,and the released process were successful without dropping off the particles. After 1,2,3 months examination,in all cases, dysphagia score kept on 0 scores, neither tumour growth and meta complications such as serious pain, fistula,immunity function damaged, marrow repress, et al. Conclusion Application irradiation stent in esophageal and cardiac cancer is safe, feasible, and effective, and its short-term clinical effectiveness was satisfaction.
