股骨粗隆间骨折两种髓外内固定方法临床对比分析

Clinlical Contrastive Analysis of Two Different Internal Fixations of Femoral Intertrochanteric Fracture

目的观察分析股骨粗隆间骨折的两种手术方法的临床疗效。方法对283例股骨粗隆间骨折分别行改良动力髋螺钉(DHS)和股骨近端解剖钢板手术内固定治疗,从手术时间、术中出血量、术后引流量、骨折临床愈合时间、术后功能恢复程度、术后并发症等方面对比分析。结果随访5个月～5年,283例股骨粗隆间骨折全部愈合,按Harris评分标准优良率达90%以上。结论改良DHS和解剖钢板内固定术均固定牢固,手术操作简单,并发症少,降低了护理难度,术后患者可以早期功能锻炼,康复良好,值得推广。

Objective To observe the effect in two different internal fixations of femoral intertrochanteric fractures. Methods Internal fixation was performed with the inproved DHS. anatomical plates of femoral upside for the 283 cases of femoral intertrochanteric fractures. The result of both groups were analyzed contrastively through the operation time,the Surgical bleeding amount,the postoperative effluent amount,the clinical cicatrize time of fracture,the recover degree of postoperative function,the postoperative syndrome etc. Results The follow-up was 5 months to 5 years. All the 283 cases of fractures healed and the hips function recovered well. Acccording to the Harris standards of grading,the excellent and good rate was over 90%. Conclusion The two methods of internal fixation proved to be firm, simple and useful with few postoperative complications, good rehabilitation and relieving nursing care.
