六君子汤加减治疗慢性阻塞性肺疾病稳定期营养不良的临床研究

Clinical study of Decoction of six noble drugs on chronic obstructive pulmonary disease in stable phase complicating with malnutrition

目的探讨中药六君子汤加减治疗慢性阻塞性肺疾病(COPD)稳定期合并营养不良的临床疗效。方法选择COPD稳定期合并营养不良、中医辨证为肺脾气虚型的患者54例,随机分为2组,对照组采用西医COPD稳定期常规疗法,治疗组在此基础上加用六君子汤治疗,疗程均为2个月。观察2组治疗前后临床疗效、体质量指数(BMI)、理想体质量百分比(NM%)、血清白蛋白(ALB)、总淋巴细胞计数(LYM)、血清前白蛋白(pro-ALB)、瘦素(Leptin)变化。结果治疗组患者临床症状、营养学和血清学指标均得到明显改善,患者血清瘦素水平显著升高,与对照组比较有显著性差异(P<0.05)。结论六君子汤可明显改善COPD稳定期患者的临床症状、营养状态、血清瘦素水平,临床疗效较好。

Objective It is to explore the curative effect of Decoction of six noble drugs on chronic obstructive pulmonary disease （COPD） in stable phase complicating with malnutrition. Methods 54 patients with stable COPD complicating with malnutrition, who were diagnosed as Deficiency of lung Qi and spleen Qi were randomly divided into two groups. The control group was treated with conventional western medicine used for stable COPD, and the treatment group was added with Decoction of six noble drugs on the basis above. Both the treatment courses were 2 months. The changes of clinical efficacy, body mass index （BMI）, percentage of ideal body weight （NM%）, serum albumin （ALB）, total lymphocyte count （LYM）, serum albumin （pro- ALB） and Leptin in both groups before and after treatment were observed. Results Clinical symptoms and indexes of nutriology and serum were significantly improved, and serum level of Leptin rose up in treatment group after treatment. Compared with control group, the differences were significant（ P 〈 0.05 ）. Conclusion Decoction of six noble drugs can obviously improve the clinical symptoms, nutritional state and serum Leptin level in patients with COPD in stable phase, so its clinical curative effect was satisfied.
