前列腺增生术后效果不满意的尿流动力学分析

Urodynamlc analysis for the causes leading to dissatisfying result after operation therapy for BPH

目的探讨尿流动力学检查鉴别前列腺增生术后效果不满意原因的意义。方法对42例前列腺增生术后效果不满意的患者，按症状不同可分为排尿困难组32例和尿失禁组10例。对两组分别行尿流动力学检查，分析其结果。结果32例排尿困难组中，逼尿肌收缩乏力8例，膀胱出口梗阻24例；10例尿失禁组中，急迫性尿失禁6例，压力性尿失禁4例。结论前列腺增生木后效果不满意的原因以梗阻和逼尿肌功能损害为主，尿流动力学检查对前列腺增生术后效果不满意的原因分析和进一步治疗具有指导意义。

Objective To discuss the causes leading to dissatisfying result after operation therapy for BPH. Methods 42 patients who had not acquired satisfying result after operation therapy for BPH were underwent the urodynamic examination. Results 42 patients were classified into two groups according to their different symptoms ： diffieuhy to void,32 cases,including 8 cases with low contractility of detrusor, and 24 cases with bladder outflow obstruction（BOO） ;incontinence, 10 cases,including 6 cases of urgency incontinence and 4 cases of stress incontinence. Conclusion We can see that the causes leading to dissatisfying result after operation for BPH are mainly the dysfunction of detrusor and BOO, and the result of urodynamie examine can guide us the causes and the next therapy.
