降落伞式胰管空肠吻合和胆肠吻合在胰十二指肠切除术的应用体会

Application of parachute pancreaticojejunostomy and cholangiojejunostomy in pancraticoduodenectomy

目的探讨应用降落伞式胰管空肠吻合和胆肠吻合在胰十二指肠切除术中预防吻合口漏的效果。方法对2004年6月至2008年9月172例采用降落伞式胰管空肠连续吻合和降落伞式连续胆肠吻合的胰十二指肠切除患者进行回顾性分析。结果降落伞式胰管空肠连续吻合时间平均11min，降落伞式胆肠吻合时间平均9min，全部病例均未出现吻合口漏，无手术死亡。结论降落伞式胰管空肠连续吻合和胆肠连续吻合，操作简便、省时、并发症少，是胰十二指肠切除术的一种有效改进。

Objective To improve the technique of pancreaticoenterostomy and cholangiojejunostomy in pancraticoduodenectomy, so as to prevent postoperative anastomotic leakage. Methods Pancraticoduodenectomy in 172 cases were performed by parachute continuous running suture of pancreaticojejunostomy and cholangiojejunostomy. Results The average time of parachute continuous pancreaticojejunostomy was 11 min, and that of the parachute cholangiojejunostomy was 9 min. There was no pancreaticoenterostomy leakage and bile leakage in all cases, and there were no deaths. Conclusion The parachute continuous running suture of pancreaticojejunostomy and cholangiojejunostomy has the advantages of easy to operate, saving time and less complications, thus it is an effective improvement of pancraticoduodenectomy.
