彩色多普勒超声在乳腺肿块中的诊断价值

Diagnostic value of color Doppler ultrasound on mammary tumors

目的:探讨乳腺良恶性肿块彩色多普勒超声图像和血流特征,提高鉴别诊断的准确性.方法:选取女性乳腺肿块患者120例,观察肿块数目、形态、边界、内部回声等,记录血流分级、最高流速(Vs)、最低流速(Vd)和阻力指数(RI),并将超声诊断与手术病理诊断结果进行对比.结果:乳腺良恶性肿块超声图像有明显不同,超声诊断结果与病理诊断结果无明显差异;乳腺恶性肿块内血流分级多为Ⅰ级～Ⅲ级,良性肿块血流分级多为0级～Ⅰ级,两者之间差异有统计学意义(P<0.05);良、恶性肿块的Vs,Vd及RI值差异均有统计学意义(P<0.05).结论:彩色多普勒超声对乳腺肿块的定性诊断具有较高的临床应用价值.

AIM： To explore the features of color Doppler ultrasound in benign and malignant mammary tumors in order to promote the diagnostic accuracy. METHODS： A total of 120 mammary tumor patients were observed in respects of the tumor number, form, border, the internal echo and so on. The distribution of the tumor blood vessels, Vs, Vd and RI were recorded. The resuits between the ultrasound diagnosis and pathologic diagnosis were compared. RESULTS： The features of malignant tumors and benign tumors were obviously different, and the difference between ultrasound diagnosis and pathologic diagnosis was not significant（ P 〉 0.05 ）. The distribution of the malignant tumor blood vessels was mainly Ⅰ - Ⅲ, and that of the benign tumor blood vessels was 0- Ⅰ ; the difference was significant （ P 〈 0.05 ）. The difference in Vs, Vd and RI between benign tumors and malignant tumors had statistical significance（ P 〈 0.05 ）. CONCLUSION： Color Doppler ultrasound has clinical value to qualitatively diagnose manmmry tumors.
