高度近视白内障小切口摘出及人工晶状体植入术

Small incision cataract extraction and intraocular lens implantation in high myopia

摘要 目的探讨高度近视合并白内障的小切口白内障摘出及人工晶状体植入术的效果。方法对35例(62眼)高度近视合并白内障行小切口白内障摘出及人工晶状体植入。眼轴长26~32.50mm,平均27.86mm,其中眼轴长≤30mm者48眼,眼轴长>30mm者14眼,观察术后视力及术中、术后并发症。结果眼轴长≤30mm的48眼中术后视力≥0.3者40眼(83.33%),眼轴长>30mm的14眼中术后视力≥0.3者6眼(42.86%),术中悬韧带部分断离2眼,后囊破裂2眼,但均植入人工晶状体。结论小切口白内障摘出及人工晶状体植入术,适合于高度近视合并白内障,但术后视力的恢复与眼轴长度、眼底黄斑功能密切相关,眼轴长>30mm者术后视力恢复较差。 Objective To study the clincal effect of small incision cataract extraction and introcular lens (IOLs)implantation in patients with high myopia. Methods The small incision cataract extraction and IOLs implantation was done on 62 cases (62 eyes) with high myopia. The axial length was 26-32.50 mm. There were 48 eyes in ≤30 mm axial length group and 14 eyes in 〉 30 mm axial length group. The postoperative visual acuity and conplications was observed. Results There are 40 eyes (83.33%) resulted in visual acuity better than 0.3 in ≤30 mm axial length group, while 6 eyes(42.86% ) in 〉30 mm axial length group, posterior capsule ruptured in 2 cases, lens suspensory ligament broken and IOLs implanted in ciliary sulcus in 2 cases. Conclusion Small incision cataract extraction and IOLs implantation was fit for high myopia with cataract. But the effect was related with axial length and function of macular. The vision recovery for eyes with axial length over 30 mm was unsatisfactory.
