可调式角度压板的研制及在股骨粗隆间骨折的临床应用

Adjustable angle clips and Development of clinical application in Intertrochanteric fracture

目的寻求一种操作简单,创伤最小,适应症更广的股骨粗隆间骨折的治疗新方法。方法自行研制一种可调式角度压板,对固定系统进行力学分析与测试,对股骨粗隆间骨折病人进行经皮穿针可调式角度压板外固定治疗。结果手术时间平均32min;术中出血<10ml。本组病例无1例死亡;无1例固定针松动及滑脱;无1例褥疮形成;出现肺部感染2例,注射抗生素治愈。髋关节功能评定:优8例,良6例,可2例,优良率达87.5%。结论斯氏针可调式角度压板固定系统符合力学原理、具有骨折断端轴向加压作用、能有效对抗髋内翻应力。"板适应针"的设计降低了经皮穿针的技术要求;斯氏针可调式角度压板固定术是一种操作最简单、创伤最小、效果肯定、适应症广的股骨粗隆间骨折的针对性治疗方法。

Objective To find a new clinical method for Intertrochanteric fracture with simple operation , minimal trauma and broader indications. Methods Development of a self-adjustable angle clips, the mechanical analysis and testing of the fixation system , 16 cases of Intertrochanteric fracture were treated with Steinmann nail and adjustable angle clips external fixation.Results The average operating time was 32 min; average bleeding 〈20 ml. no case died , no case of fixed-loose and slip, no case of bed sores formed and two cases of lung infection. Functional recovery of hip joint was excellent in 8 cases, good in 6 cases, and general in 2 cases.Conclusion Steinmann nail and adjustable angle clips fixation system meets mechanics thery, with stump axial compression between fractures, can fight against hip varus stress effectively. The design, ＂Plate to nail＂ reduced the operating technical requirements; Steinmann nail and adjustable angle clips fixation is a targeted Treatment of Intertrochanteric fracture with a very simple operation, the smallest trauma, the sure effect, the more wider indications.
