摘要
目的:探讨检测孕妇血清妊娠相关血浆蛋白A(PAPP-A)和游离β-人绒毛促性腺激素(β-hCG)水平,在预测异位妊娠及稽留流产中的诊断价值。方法:选取早期妊娠孕妇164例,其中正常妊娠组80例、异位妊娠组36例、稽留流产组48例,用时间分辨荧光法测定孕妇血清PAPP-A及游离β-hCG的浓度。结果:在正常妊娠组、稽留流产组及异位妊娠组妇女PAPP-A及游离β-hCG的中位值呈递减趋势,且各组间差异有统计学意义(P<0.001);与正常组比较,稽留流产组及异位妊娠组的血清游离β-hCG和PAPP-A的中位值水平差异有统计学意义(P<0.001);与稽留流产组比较,异位妊娠组血清游离β-hCG和PAPP-A的中位值水平下降,差异有统计学意义(P<0.001)。结论:孕早期母血清筛查PAPP-A和游离β-hCG指标对稽留流产及异位妊娠有一定预测价值。
Objective： To explore the diagnostic values of pregnancy - associated plasma protein A （ PAPP - A） and free β - HCG in the forecast of missed abortion and ectopic pregnancy. Methods ： 164 women with early pregnancy were enrolled in this study, of which 80 cases with normal pregnancy, 36 cases with ectopic pregnancy, and 48 cases with missed abortion. The concentrations of pregnancy serum PAPP - A and freeβ - HCG were detected by time resolved fluoroimmunoassay. Results ： The median levels of PAPP - A and freeβ- HCG decreased gradually in the normal pregnancy group, missed abortion group and ectopic pregnancy group, respectively, with statistical significance （P 〈 0. 001 ）. Compared with the normal group, the differences of serum levels of PAPP - A and free 13 - HCG between the missed abortion group and ectopic pregnancy group were statistically significant （P 〈0. 001 ）, the serum levels of free β - HCG and PAPP - A in the ectopic pregnancy group were lower than the missed abortion group （P 〈 0. 001 ）. Conclusion： The determination of serum PAPP - A and freeβ - hCG levels have some predictive value for missed abortion and ectopic pregnancy.
