鼻内镜辅助射频热凝鼻后下神经和筛前神经治疗变应性鼻炎对鼻腔通气功能的影响

Effect of endoscopic resection of posterior nerve and anterior ethmoidal nerve by radiofrequency on nasal ventilation in patients with allergic rhinitis

摘要 目的:探讨射频阻断鼻腔副交感神经治疗常年性变应性鼻炎的疗效。方法:常年性变应性鼻炎(PAR)患者77例随机分为两组:观察组40例,应用鼻内镜下射频热凝鼻后下神经、筛前神经及下鼻甲治疗;对照组37例,应用鼻内类固醇激素治疗。随访6月以上,采用症状体征积分和鼻声反射检查两项指标对远期疗效进行对比。结果:观察组总有效率为90.0%(其中显效27例),明显高于对照组(62.2%,其中显效9例),P<0.01。鼻声反射检查示鼻腔最小截面积和鼻阻力较术前有改善(P<0.05)。结论:鼻内镜辅助下射频热凝鼻后下神经和筛前神经治疗PAR,不仅对喷嚏、鼻痒等症状控制作用显著,对于鼻腔通气功能的改善亦较明显。 Objective: To investigate the effect of resecting posterior nerve and anterior ethmoidal nerve by radiofrequency under endoscopy on pereunial allergic rhinitis(PAR). Method:Forty PAR patients were treated by endoscopic radiofrequency, and 37 PAR patients were treated by nasal corticosteroids as control. All patients had been followed up more than 6 months. The outcomes were evaluated by score of symptom, nasal inspection and nasal acoustic reflection ( NAF). Result: 37 patients (00.0%) showed improvemont after the radiofrequency treatment, and remarkably better than the control group which showed 62.2% improvement( P 〈 0.01 ). The NAF result showed that Nasal Minimal Cross - seetional Area ( NMCA} and Nasal Resistance( NR) demonstrated significant improvement after the radiofrequency therapy( P 〈 0.05 ). Conclusion : Resection of nasal posterior nerve and anterior ethmoidal nerve by radiofrequency under endoscopy could remarkably improve the symptom of PAR, not only for sneezing and rhinorrhea, but also nasal obstruction.
