核磁共振快速序列在OSAHS上气道阻塞定位中的作用

摘要 目的:介绍超快速核磁共振成像(简称超快速MRI)诊断阻塞性睡眠呼吸暂停综合征(OSAHS)患者的上气道阻塞部位的应用体会。方法:22例OSAHS患者和20例健康成人在清醒时与自然睡眠状态时,应用超快速MRI检查,获得上气道正中矢状面的系列成像,制成电影形式,对照两者上气道的狭窄点和阻塞点情况。结果:在清醒状态下,9例(占41%)OSAHS患者有腭咽部9个狭窄点;当患者睡眠时,22(占100%)例患者有45个阻塞点,其中3例患者仅腭咽区有一个阻塞点,15例患者在腭咽及舌咽区有阻塞点,4例在腭咽、舌咽、喉咽区均有阻塞。OSAHS患者清醒时上气道的狭窄与睡眠时上气道的阻塞的符合率为20%。而在正常对照组中无阻塞发现,有8例出现腭咽部狭窄。结论:超快速MRI成像是定位OSAHS患者上气道阻塞部位的有效方法,OSAHS患者清醒时气道的狭窄和睡眠时气道阻塞并不完全一致。
