摘要
目的探讨激素致股骨头坏死(ONFH)股骨头局部血管内皮生长因子(VEGF)与骨形态发生蛋白-2(BMP-2)的表达情况及意义。方法收集经全髋置换术取下的激素性ONFH股骨头标本35例作为实验组,新鲜股骨颈骨折股骨头标本32例作为对照组。所有标本经固定、脱钙后,制成切片,光镜观察其病理变化,并运用免疫组化技术分别对其VEGF和BMP-2的表达情况进行检测。结果实验组组织结构紊乱,破碎,骨小梁稀疏、不完整,有大量的空骨陷窝,而对照组骨小梁相对较完整、排列整齐,骨细胞清晰可见,偶见空骨陷窝。实验组VEGF和BMP-2阳性表达的强度及面积均明显低于对照组,结果有统计学意义(P<0.05)。结论激素性ONFH股骨头局部VEGF和BMP-2的表达降低,极有可能是导致其自身修复能力不足的重要原因,增加股骨头局部VEGF与BMP-2的表达可能有助于ONFH的修复。
Objective To explore the change and its significance of the expression of vascular endothelial growth factor (VEGF) and bone morphogenetic protein- 2 (BMP- 2) of the local femoral head in steroid- osteonecrosis of femoral head (steroid- ONFH). Methods 35 femoral head samples of steroid - ONFH was collected as the experimental group and fresh 32 samples of femoral heads of femoral neck fracture as control group, overall examples were collected from total articular replacement arthroplasty. The bone tissues were made into general sections after the process of immobility and decalcification. Pathological change was observed by using optical microscope and detects the expression of VEGF and BMP - 2 in femoral head through making use of immunohistochemistry technique. Results The organization structure of experimental group was disorganized cracked and the bone trabecula was rarefactive and non - intact and there were a great number of empty lacuna in bone trabecula. While there was the reverse situation in the control group. The intensity and area of positive expression of VEGF and BMP - 2 in femoral head of the experimental group were lower than that of control group obviously. The result shows statistical significance ( P 〈0. 05 ). Conclusion The decrease of expression of VEGF and BMP - 2 in femoral head of steroid - ONFH is important reason that affect its rehabilitation and reconstruction. It may be of benefit to the reparative process of the femoral head necrosis if the expression of VEGF and BMP - 2 will increase in the femoral head.
