酒依赖伴发精神分裂症的临床特征对照研究

Research of clinical characteristics of alcohol dependence accompanied with schizophrenia.

目的探讨酒依赖伴发精神分裂症的临床特征。旨在为临床鉴别诊断提供参考。方法采用回顾性调查方法,将患者分为酒依赖伴发精神分裂症组及酒精所致精神障碍组,并对两组饮酒情况、家族史、诱因、临床特征、实验室检查等资料进行比较。结果两组在饮酒量、家族史、诱因、幻觉、妄想症状持续时间、神经系统症状、脑电图、疗效方面有显著差异。结论酒依赖伴发精神分裂症饮酒量相对较少,家族史阳性率高,无明显诱因,幻觉妄想症状持续时间较长,伴有分裂症其它特征性症状,自知力缺乏,智能无明显缺损、无明显神经系统阳性体征,脑电图多数正常,疗效相对较差,可与慢性酒中毒相鉴别。

Objective To offer accurate bases for differential diagnosis of by discussing clinical characteristics of alcohol dependence accompanied with schizophrenia in clinic. Methods A retrospective survey methodology was adopted to investigate two groups of patients of alcohol dependence accmpanied with schizophrenia and alcohol - induced mental disorders, each group of which includes 42 cases, the data of drinking, family history, inducements, clinical characteristics, and laboratory examinations were compared. Results There were significant differences between the two groups of drinking quantities ,family history, inducements, the duration of symptoms of hallucination and deliria, symptoms of the nervous system, EEG, and curative effect. Conclusion Comparing to patients of chronic alcohol intoxication, the patients of alcohol dependence accompanied with schizophrenia have less drinking, higher rate of positive family history, no obvious inducements, longer duration of hallucination and deliria, other characteristic symptoms of schizophrenia, no insight, no obvious amentia, no obvious positive signs of the nervous system, normal EEG,poorer curative effect.
