微创经皮肾输尿管镜钬激光碎石术治疗嵌顿性输尿管上段结石 被引量:1

摘要 目的探讨微创经皮肾输尿管镜钬激光碎石术治疗嵌顿性输尿管上段结石的可行性与疗效。方法肾盂集合系统造影后用18G穿刺针穿刺肾中上盏后组盏,从针鞘中置斑马导丝入肾收集系统或入输尿管。通道逐渐扩张至Fr16-18,再留置16-18Fr Peel-away薄鞘,建立微造瘘通道。Wolf F8/9.8输尿管镜在直视下进入肾盂。若合并肾结石则先以钬激光击碎并用灌注冲洗的方法取出结石,然后进入输尿管,以扩张鞘抵住结石,钬激光击碎结石,同样用灌注冲洗的方法将结石取出。结果经皮肾输尿管镜的一期成功率96.3%(52/54),2例脓肾改行Ⅱ期经皮肾输尿管镜并顺利取出结石。全部病例随访2-28个月,未见结石复发,肾积水有不同程度的减轻。结论微创经皮肾输尿管镜钬激光碎石术对治疗嵌顿性输尿管上段结石具有安全、有效、微创、恢复快等优点,可作为输尿管上段嵌顿结石的首选治疗方法。
