老年肺癌患者MGMT、LRP、HIF-1α表达与临床预后关系组织芯片研究

摘要 目的:探讨老年肺癌MGMT、LRP、HIF-1α表达与临床预后之间的关系。方法:应用组织芯片技术和免疫组织化学方法检测34例老年肺癌MGMT、LRP、HIF-1α的表达,并与临床预后资料进行相关性分析。结果:MGMT、LRP、HIF-1α在老年肺癌的阳性表达率分别为47%(16/34)、58%(19/34)、64%(22/34);MG-MT阳性表达与组织学类型、TNM分期密切相关(P<0.05),与肿瘤体积、淋巴结转移无关(P>0.05);LRP阳性表达与TNM分期、淋巴结转移密切相关,与组织学类型、肿瘤体积无关(P>0.05);HIF-1α阳性表达与组织学类型、肿瘤体积密切相关,与TNM分期、淋巴结转移无关(P>0.05)。经多因素Cox比例风险模型分析显示:TNM分期、肿瘤组织类型及MGMT、HIF-1α阳性表达与肺癌患者术后生存率有关(P<0.05)。结论:TNM分期、肿瘤组织类型及MGMT、HIF-1α可作为老年肺癌的独立预后指标。 Objective: To investigate the relationship between MGMT,LRP, HIF-1α's expression and prognosis in old-aged lung carcinoma. Methods: The expressions of proteins MGMT,LRP, HIF-1α were detected by immunohistoehemistry method and tissue microarray technology in 34 cases of old-aged lung carcinoma and the relationship between the expressions of MGMT,LRP, HIF-1α and clinical prognosis was analysed. Resuits: The expression ratios of MGMT ,LRP, HIF-1α were 47 % (16/34) ,58 % (19/34), 64 % (22/34) respectively;The expression of MGMT was associated with histological grading, TNM staging(P〈0. 05), but not associated with tumor volume and lymph node metastasis(P〉0. 05);the expression of LRP was associated with TNM staging and lymph node metastasis(P〈0.05), but not associated with tumor volume and histological grading(P 〉 0. 05) ; the expression of HIF-1α was associated with tumor volume, histological grading (P〈 0.05) ;but not associated with TNM staging and lymph node metastasis(P〉0.05) ;In the multivariate Cox analysis,survival rate was correlated with TNM staging, histological grading, MGMT and HIF-1α (P〈0. 05). Conclusion:TNM staging, histological grading,the expression of MGMT, HIF-1α can be regarded as independent index of old-aged lung carcinoma respectively.
