The Clipping of Intracranial Aneurysm with Endoscope-assisted Microneurosurgery

摘要 颅内动脉瘤的微侵袭治疗是动脉瘤外科的新方向。内镜辅助显微神经外科弥补单纯显微外科的一些不足。利用内镜,手术医师能在较小的显露范围内看清动脉瘤的各个方向和周围结构,减少对正常脑组织的牵拉;内镜下可以看清载瘤动脉上的重要穿支血管,确定夹闭位置是否正确,瘤颈夹闭是否完全,动脉瘤夹是否压迫周围的重要的神经及血管;在个别复杂的动脉瘤病例中,只有在内镜视野内才能夹闭。 Microinvasive treatment of intracranial aneurysms is a new direction of aneurysm surgery. Treating by endoscope-assisted microsurgery retrieves some deficiency of simple microsurgery. Using endoscope, operative doctors can observe all directions and surrounding structures of aneurysm, and reduce the pull or oppression of normal brain tissue ; Endoscope can be not only used to identify the important branch artery, but also to identify if the occlusion site is right or not, if the tumor neck is completely clipped or not,if the aneurysm clip does oppress the important nerves and vessels or not. As for the complex cases of intracranial aneurysm,it can be clipped only under endoscope.
