酶联免疫吸附测定法与化学发光微粒子免疫分析法检测广州地区儿童HBsAb结果分析

Analysis of the results of ELISA and CMIA methods used in detecting serum HBsAb of children in Guangzhou area

目的:比较化学发光微粒子免疫分析法(CMIA)与酶联免疫法(ELISA)检测1 472例广州地区儿童HBsAb结果的差异,并探讨HBsAb的阳性率与儿童年龄、性别的相关性。方法:收集广州地区1 472例0～6岁儿童的血清,同时采用ELISA法与CMIA法进行HBsAb的定性及定量检测,采用统计软件SPSS 12.0比较两种方法的检测结果,并分析不同性别组和不同年龄组儿童间HBsAb的阳性率差异。结果:两种方法检测1 472例儿童血清HBsAb,存在57例的结果判读不一致。以CMIA法判定(HBsAb≥10mIU/ml),HBsAb的阳性率高达94.09%,而以ELISA法判定(As/CO≥2.1),HBsAb的阳性率为91.17%,两种检测方法检测的HBsAb的阳性率差异具有显著性意义;不同性别组儿童的血清HBsAb的阳性率差异无显著性意义;不同年龄组儿童的血清HBsAb的阳性率差异具有显著性意义,年长组较年幼组儿童的血清HBsAb的阳性率低。结论:儿童血清HBsAb的阳性率与性别无关,但随年龄增长而降低。采用CMIA法检测血清HBsAb有助于我们更加及时、准确地了解乙肝疫苗免疫后的效果。

Objective： To compare HBsAb measurements of 1 472 children serum samples between chemiluminescent micropartiele immunoassay （CMIA） and ELISA, and to analyze if the positve rate of HBsAb is related with gender or age. Methods ： Serum samples from 1 472 children who birthed in Guangzhou area after 2002 were collected, and were simultaneously tested for HBsAb by CMIA and ELISA. Compare results of these two commercially available HBsAb assays, and analyze if the positve rate of HBsAb is related with gender or age by SPSS 12.0. Results： These two methods gave related but not identical results. Of 1 472 samples, 57 cases showed inconsistent results. According to CMIA （HBsAb ≥ 10mIU/ml） , the positve rate of HBsAb arrived 94. 09% , however, it is 91.17% according to ELISA. There are significant differences between the positve rates of HBsAb. when analyzing the relation between the positve rates of HBsAb and children＇ s gender or age, results showed that significant differences of positve rates exist in different age groups of children （higher positve rate in young group）, not in different gender groups. Conclusion： The positve rate of HBsAb has nothing to do with gender, but may decrease with age. CMIA can help us to gain a more timely and accurate understanding of the immunization status against HBV of children and other peple.
