摘要
目的比较Ⅰ型与Ⅱ型精神分裂症患者治疗前、后脑甲状腺功能状态及其中枢调节方面的差异。方法电抽搐治疗(ECT)Ⅰ型精神分裂症、氯氮平治疗Ⅱ型精神分裂症;在治疗前、后测定促甲状腺激素(TSH)含量、简明精神病评定量表(BPRS)、阳性症状评定量表(SAPS)、阴性症状评定量表(SANS)。结果Ⅰ型精神分裂症患者脑脊液TSH值略高于Ⅱ型;ECT对Ⅰ型患者脑脊液TSH水平有双向调节功能,调节效应好;氯氮平对Ⅱ型患者脑脊液TSH水平的调节功能和效应低下;治疗前后脑脊液TSH增(减)值Ⅰ型较Ⅱ型大(P<001)。
Subjective To compare the differences of cerebrothyroid function and central regulation between type I and type II schizophrenias before and after treatment. Methods Treat type I schizophrenia with electric convulsive therapy (ECT) and type II with clozapine, measure the concentration of thyroid-stimulating hormone (TSH) and evaluate BPRS, SAPS and SANS before and after the treatments. Results The TSH levels of cerebrospinal fluid (CSF) from the type I is a little higher than that from type II. ECT has two-way regulatory function and good efficacy on the TSH levels of CSF from the type I. Whereas clozapine has low regulative function and efficacy. The average increment level (0.70±0.29) and decrement level (-0.70±0.15) of TSH of CSF from the type I are more than those from type II (0.34±0.28, -0.21±0.11, P<0.01). Conclusions This result suggests that there are obvious differences of cerebrothyroid function and central regulation between type I and II.
