引起剖宫产术中出血的原因及防治探讨

The Causation and Prevention of the Bleeding during Cesarean Section.

目的探讨引起剖宫产术中出血的原因及预防措施。方法回顾性分析2004年1月至2007年12月发生剖宫产术中大出血50例患者的资料。结果出血原因依次为:子宫收缩乏力、胎盘因素、子宫切口裂伤、凝血机制障碍,针对各种原因的止血方法的同时,注重全身综合治疗,及时输液输血,维持生命体征稳定。50例均取得了明显的止血效果,保留了年轻妇女的子宫;2例胎盘植入面积大,行子宫次全切除术;1例DIC行全子宫切除术。结论产后出血比较常见;与许多因素有关,最主要的原因是子宫收缩乏力及胎盘粘连,应对出血的采取针对性相关预防措施减少出血发生。

Objective To discuss the causation and prevention of the bleeding during cesarean section. Methods Retrospectivly analysis the information of 50 patients who suferred massive bleeding during cesarean section fromJan.2004 to Dec.2007. Results The resons caused bleeding are followed by： weak uterine contraction, placenta factors, uterine incision laeration, obstacles in clotting mechanism, besides taking various methods to stop bleeding,at the same time we need to focus on comprehensive body treatment, taking infusion and blood transfusion on time to maintain the stability of vital signs. 50 cases were made clear effect in stoping bleeding and retain the uterus of the yong women.But among them, 2 cases with big area placental implantation, got subtotal hysterectomy, 1 case DTC, got total hysterctomy. Conclusions Postpartum hemorrhage is a common Phenomenon, it is related to many factors, the main reason is weak uterine contraction and Placental adhesion. To deal with bleeding, we need,to take specific preventive measures to reduce the occurrence of bleeding.
