摘要
目的探讨大脑内静脉在经胼胝体-穹隆间手术入路中的意义。方法利用15例人尸颅脑标本,在显微镜下模拟经胼胝体-穹隆间入路,并对大脑内静脉及其相关属支进行解剖,观察并测量相关的数据。结果大脑内静脉在解剖学上存在较大的变异,其明显变异的属支即透明隔静脉外径与大脑内静脉前部的外径差别不大。结论大脑内静脉有些变异使得大脑内静脉或其较大的属支成为选择经胼胝体-穹隆间手术入路时进入第三脑室的障碍,这些情况应当引起神经外科医师的注意,以减少术中不必要的出血和术后并发症。
Objective To discuss the significance of internal cerebral veins in transcallsal-interforniceal approach. Methods Fifteen formalin fixed heads were applied to stimulate the transcallsal-interforniceal approach under a microscope. The internal cerebral vein and its related branches were dissected, measured and studied. Results There was a major variation in the anatomy of the vein, i.e. the difference between the external diameter of the obviously variant spetum pellucidum vein and the anterior part of the internal vein was not marked. Conclusion Some variation of the internal cerebral vein and its bigger branches result in obstacles to approach through transcallsal-interforniceal to the third ventricle, an attention should be paid on this point to avoid unnecessary bleeding and post-operative complications.
