不同剂量布托啡诺术后硬膜外病人自控镇痛效应的比较 被引量:9

摘要 目的研究不同剂量布托啡诺复合0.2%左旋布比卡因行术后硬膜外腔病人自控镇痛(PCA)的临床效果和不良反应。方法选择子宫全切手术患者120例,ASAⅠ级或Ⅱ级,根据术后镇痛药物的不同,所有患者随机分为4组(n=30),药物配置为:B1组:采用0.2%左旋布比卡因+0.04%布托啡诺;B2组:0.2%左旋布比卡因+0.06%布托啡诺;B3组:0.2%左旋布比卡因+0.08%布托啡诺;M组(对照组)0.2%左旋布比卡因+0.01%吗啡。取T11-12与L3-4椎间隙穿刺留置硬膜外导管,2%利多卡因3~5 mL试验剂量后阻滞平面达T6,术毕留置T11-12行术后PCA。4组麻醉方法相同,即其PCA采用持续输注+PCA模式(LP)持续输注1 mL/h,Blous 1mL/次,锁定时间为10 min。采用视觉模拟评分(VAS)评价镇痛效果,分别记录术后4,8,12,16,24 h VAS评分,并同时记录患者自控镇痛泵的按压次数D1/D2比值以及恶心呕吐、瘙痒等不良反应的发生。结果4组患者基本情况相似(P>0.05);术后4,8,12,16,24 h 4组均差异无显著性(P>0.05),术后8,12,16 h B2组、B3组与M组的VAS评分均显著低于同期B1组(P<0.01);而B2组、B3组与M组各时间段VAS记分相似。各组术后恶心呕吐发生率分别是3.33%,6.67%,20%,26%;瘙痒发生率则为0%,3.33%,6.67%,10%;无低血压和呼吸抑制情况。结论经腹子宫切除患者术后硬膜外PCA以LP模式给药时0.2%左旋布比卡因复合0.06%布托啡诺效果最佳,且不良反应较少,可供临床选用。
