摘要
目的探讨直肠癌术后局部复发的预防和治疗问题。方法回顾性分析1997年1月至2007年12月所收治的46例直肠癌术后局部复发患者的临床资料。结果保肛手术后局部复发34例,占73.91%。共40例行再次手术治疗,其中36例行复发肿瘤切除,随访33例,1年生存率77.78%,3年生存率44.44%,5年生存率13.89%。结论慎重选择保肛手术,根治术遵循直肠全系膜切除术(TME)原则是减少复发的两个主要方面,术后的密切随访可早期发现肿瘤复发。明确直肠癌术后局部复发后应争取手术治疗并进行以手术为主的综合治疗仍可取得较好的治疗效果,晚期患者放疗可缓解症状。
Objective To investigate the prevention measure and therapeutic effects for postoperative local recurrent rectal cancer. Methods From 1997 to 2007, the clinical data of 46 patients with postoperative local recurrent rectal cancer were retrospectively analyzed. Results There were 34 patients with local recurrence after sphincter preservation operation（73.91% ）. 40 patients underwent operation,36 recurrent tumors were resected. The 1 year survival rate was 77.78 %, 3-year survival rate 44.44 % ,and 5-year survival rate 13.89~. Conclusion Follow- up should he performed after surgery and synthetic therapy should be strived for the pa tients with postoperative local recurrent rectal cancer. Radiotherapy might relieve the pain for the terminal cancer.
