锁骨下静脉穿刺置管并发症的分析及防治

Cause analysis and preventing method of Complication Subclavian vein Catheterization

目的:探讨锁骨下静脉穿刺置管并发症发生的原因和防治方法。方法:密切观察450例行锁骨下静脉穿刺置管病人并发症的发生,并对其原因进行分析,提出相应的防治方法。结果:其中57例出现并发症,发生率12.67%,包括气胸、血肿、心率失常、感染、导管堵塞、异位输液等。结论:操作者要熟悉锁骨下静脉周围的解剖结构,熟练掌握穿刺技术,操作轻柔,遵守无菌技术操作原则,掌握置管的适应证、禁忌证,熟悉穿刺置管常见的并发症及其发生的原因,积极采取相应的防治措施。

Objective： To investigate the causes and the prevention and treatment of complication of Subclavian vein Catheterization. Methods： Based on close observation and analysis on 450 patients with Subclavian vein Catheters, put forward some relerant treatment methods to the complication, Results： Among these 450 patients, 57 cases got the complication, and the ratio was 12.67%. They showed such symptoms or phenomera as pneumothorax, haematoms, arabyhmia, infection, catheter obstruction, and transpesal trsnsfusion etc. Conchusion： The actor need to be familiar with the anatomical structure, and to be proficient in the operation. And they must strictly implement the operating norms and asepsis technique. Master the indication and Contraindication of puncture, and must be familiar with the causes of the complication and take conespending measures of prevention and cure in time.
