1630例高血压住院患者的降压药物使用分析

目的分析某院2006~2007年抗高血压药的药物利用情况,为临床合理用药提供依据。方法采用回顾性调查分析方法,以＂限定日剂量（DDD）＂作为药物利用动态客观指标,药物利用指数（drug utilization index,DUI）作为分析判别指标,对某院心血管内科2006~2007年1630例高血压病患者使用过的所有抗高血压药物进行分析统计。结果2006与2007年DDDs与销售金额排序前2位的药物均为血管紧张素转化酶抑制剂（ACEI）和钙离子拮抗剂。2年中DDDs排序前10位的药物品种中有9种相同。同时DDDs排序前10位的药物中,除了依那普利、美托洛尔及螺内酯外,其余均进入了品种销售金额前10位。2006年单个药物用药频度居首位的是硝苯地平控释片,2007年为福辛普利。且大部分抗高血压药物的DUI值≤1.0。某院2007年抗高血压药物中国家基本药物的应用率高于2006年。结论某院抗高血压药物中,DUI值≤1.0的药物超过85%,使用基本合理。

Objective To analyze the use of antihypertensives in a hospital, and provide evidence for rational drug use. Methods Antihypertensives uesd in 1630 patients during 2006-2007 were retrospeeively analyzed, with ＂defined daily dose＂ as the dynamic objective index of medical use and ＂drug utilization index＂ as the discriminant analysis. Results In 2006 and 2007, leading the list of the DDDs and consumption sum were calcium antagonists and angiotensinconverting enzyme inhibitor （ACEI）. In the 2 years, 9 drugs were the same among the top 10 of DDDs, and the same drugs were in the top 10 list of consumption except enalapril, metoprolol and spironolactone. Nifedipine controlled release dominated DDDs in 2006, and fosinopril in 2007. DUI of most antihypertensives was less than 1.0. National essential drugs of antihypertensives were used more in 2007 than in 2006. Conclusion More than 85% of the antihypertensives whose DUI was≤1.0 fairly rational used in the hospital.
