引导骨再生在人工牙骨内即刻种植中的应用 被引量:2

摘要 目的:在人工牙即刻种植术中将多孔骨无机质以及复合自体骨植入种植体周骨缺损区,覆以可吸收膜的引导骨再生技术(Guided Bone Regeneration,GBR)进行骨内即刻种植,以探讨即刻种植术中引导骨再生的应用,以促进种植体骨融合,确保种植成功和良好的临床效果。方法:在新鲜拔牙创内即刻植入ReplaceTM根型螺纹纯钛种植体,采用与植体类型配套的扩骨器的骨挤压法制备种植体窝,在种植体与拔牙窝之间的骨缺损区植入Bio-OssTM颗粒多孔骨无机材料以及复合自体骨植入,覆以Bio-GideTM可吸收膜。结果:所有13例即刻种植18枚植体,均达到临床和放射学上的骨融合,植骨材料及自体咀嚼粘骨膜瓣也正常愈合。13例患者即刻种植义齿修复临床效果满意。结论:即刻种植中种植体周的骨质缺损区不大,骨量不足的情况也较轻,较适合多孔骨无机质膜下植入的应用,能达到充填骨缺损、引导骨生成、与种植体和自体骨相骨性结合的作用。本组病例随访时间短,虽远期成功率无从谈及,但种植体及植骨材料的成活,软、硬组织的成功保护,较满意的临床效果,增强了术者对即刻种植临床应用的信心,并积累了宝贵的经验。可以相信,本组病例即刻种植中所采用的骨引导再生技术是成功的。 Objective: Immediate implant placement after tooth extraction is becoming a common procedure in implant - supported oral rehabilitation. However, lack of hard tissue and primary full flap closure can jeopardize final results. Method: 18 tapered implants were placed in 13 patients using immediate implant mary soft tissue closure over extraction sites. Result: All implants result in predictable and asymptotic healing procedure and display a clinical and radiographic features of osseointegration. Conclusion: Techniques that provide for the preservation of both bone and soft tissue while enhancing the esthetic results around implants and the advantage of GBR was discussed.
