摘要 目的:观察控制性肺膨胀(sustained inflation SI)对急性肺损伤(ALI/ARDS)患者的影响。方法:60例ALI/ARDS患者均在常规治疗基础上予呼吸机辅助呼吸,实行肺保护性通气,如指氧饱和度(SpO2)低于90%,予控制性肺膨胀治疗,于治疗前后监测潮气量(VT)、气道平台压(Pplat)、吸入峰值压(Ppeak)、平均气道压(Pmean)、呼吸系统顺应性(Crs)、气道阻力(R)及动脉血气分析。结果:应用控制性肺膨胀后,患者VT、Crs增加,Ppeak、Pplat、Pmean、R下降,动脉血气分析改善。结论:控制性肺膨胀在肺保护性通气的基础上可改善ALI/ARDS患者的氧合。 Objective: To observe the effect of sustained inflation to ALI/ARDS patients. Method: 60 patients with ALI/ARDS were all given mechanical ventilation adding to conventional therapy. And SpO2 in the patients was 90 percents below were given sustained inflation. Then observed the changes of VT, Pplat, Ppeak, Pmean, Crs, R and SpO2 between prior - treatment and post - treatment. Result: After sustained inflation, patients' VT and Crs increased; Pplat, Ppeak, Pmean and R decreased; arterial blood gas analysis showed oxygenation ameliorated. Conclusion: Sustained inflation can ameliorate oxygenation of patients with ALI/ARDS.
