乳腺癌保留乳头乳晕复合体改良根治术的可行性

The Analysis of Feasibility in Nipple-sparing Mastectomy

目的:研究乳腺癌乳头乳晕复合体(nipple-areolar complex,NAC)的受累情况,分析保留乳头乳晕复合体改良根治术的可行性。方法:对275例乳腺癌标本的NAC采用横断多层面取材方法进行病理检查,用统计学方法分析肿瘤大小、距NAC的距离、分期、病理类型与NAC受累的关系。结果:275例中NAC受累25例,受累率9.0%。肿瘤大小、距NAC的距离与NAC受累明显相关(P<0.05),与病理类型、淋巴结转移无关。结论:对早期乳腺癌,如果无乳头乳晕复合体受累的临床征像,可行保留乳头乳晕复合体改良根治术。

Objective：To investigate the prevalance of nipple-areolar complex （NAC） in breast cancer and analyse the feasibility of nipple-sparing mastectomy. Methods：Multiple cross sections of NAC from 275 breast cancer specimens were examined to find out the rate of NAC involvement. The relation between tumor size, the distance of tumor to NAC, pathological type of tumor and the involvement of the NAC was analysed statistically. Results： Twenty-five of the 275 cases were found out NAC involvement（25/275）, The relationship between tumor size, the distance of tumor to NAC, and the involvement of the NAC was statisyic significance（P〈0. 001）. there was no relation between pathological type of tumor and axillary lymph node. Conelusion： Nipple-sparing masteetomy should be aceecpted if early stage breast cancer without clinical sign of NAC involvement.
