青少年鼻腔结构异常的鼻内镜手术探讨

Study on endoscopic sinus surgery for abnormal anatomy of nasal cavity in adolescents

目的探讨鼻内镜手术治疗青少年鼻腔结构异常的适应证及其疗效。方法对37例鼻腔结构异常的青少年患者（年龄11～17岁，平均14．6岁）实施鼻内镜外科手术，主要行鼻中隔成形术，同时治疗影响鼻窦炎发病的下鼻甲肥大、泡状中鼻甲及腺样体肥大等相关疾病。结果经过随访3个月～2年，手术后鼻塞、流脓涕、头痛等症状消失29例（78．4％），好转5例（13．5％），无效3例（8．1％）。其中1例存在鼻腔粘连，再次行鼻内镜处理。结论经鼻内镜鼻中隔成形术结合同时处理影响鼻窦炎发病的相关因素，对青少年鼻腔结构异常是一种行之有效的治疗方法。

Objective To evaluate the operative plan and the outcomes of endoscopic sinus surgery （ESS） for adolescents with abnormal anatomy of the nasal cavity. Methods The data of 37 patients （aged 11-17 years, mean 14.6 years） with abnomal anatomy of the nasal cavity who underwent ESS were retrospectively studied. Septorhinoplasty was performed on the patients under intranasal endoscopic procedures. Conchoplasty, adenoidectomy and related disorders were performed simultaneously according to nasal cavity abnormality. Results The patients were followed up for 3-24 months after ESS. 29 cases （78.4%） were free from symptoms, 5 cases （13.5%） felt relieved, and 3 cases （8.1%） were found ineffective. Conclutions Our results suggest the effect of endoscopic septorhinoplasty combined with the management of sinusitis-related factors on abnormal anatomy of the nasal cavity among adolescents.
