天丹降压颗粒逆转高血压左心室肥厚及改善舒张功能的临床研究

Clinical research of Tiandan Jiangya granule on reversal of left ventricular hypertrophy and improvement of diastolic function

目的考察天丹降压颗粒剂对高血压病靶器官的保护作用。方法精选13味中药制成天丹降压颗粒剂,治疗38例肝阳上亢、痰瘀互阻型高血压病左心室肥厚伴舒张功能障碍的患者,与卡托普利治疗的37例作对照观察,治疗6个月后统计疗效。结果两组治疗后血压值均明显下降,差异有统计学意义﹙P<0.01),但组间疗效比较差异无统计学意义(P>0.05);两组间证候疗效差异有统计学意义﹙P<0.01);反映左心室肥厚的指标LVID、IVST、LVPWT、LVMI两组治疗前后差异有统计学意义(P<0.05～0.01),但组间疗效比较差异无统计学意义(P>0.05);反映左室舒张功能的指标PVA、PVE、A/E、IRT两组治疗前后比较差异有统计学意义(P<0.01),组间疗效比较PVE、PVA、A/E差异无统计学意义(P>0.05),IRT差异有统计学意义﹙P<0.05﹚。结论天丹降压颗粒剂能降低血压,逆转高血压病左心室肥厚,改善舒张功能,在保护靶器官方面有一定的应用前景。

Objective To investigate the protective effect of Tiandan Jiangya granule on the injury of target organ of hypertension. Methods The granule was prepared with 13 kinds of Chinese drugs. The 75 patients with hypertension, accompanied with left ventricular hypertrophy and diastolic function defect of the heart, in syndromes of upper hyperactivity of liver-yang and of interconnected phlegm and blood stasis were randomly divided into control group, n=37, treated with Captopri, and treatment group, n=38, treated with Tiandan Jiangya granule. Results t The differences of curative effect in the two groups before and after the treatment were obviously significant（P〈0.01）, but the difference between the two groups was not significant（P〉0.05）. The difference of curative effect in syndromes between them was very marked（P〈0.01）; the index differences of LVID, IVST, LVPWT and LVMI, reflecting the left ventrieular hypertrophy, before and after treatment in the two groups were very marked （P〈0.05 or P〈0.01）, but they were not significant between the two groups （P〉0.05）; the index differences of PVA, PVE, A/E and IRT, reflecting the diastolic function of left ventrieular, before and after treatment in the two groups were very marked （P〈0.01）, but the differences of PVA, PVE and A/E between the two groups were not significant（P〉0.05）, and the difference of index IRT was very obvious（P〈0.05）. Conclusion Tiandan Jiangya gramule is effective in clinic to decrease the blood pressure, reverse left ventricular hypertrophy of hypertension, improve diastolic function and prevent the target organ.
