老年脑白质疏松症TCD及相关因素分析

TCD and correlation factor analysis in senile leukoaraiosis

目的了解脑白质疏松症（ＬＡ）患者脑血流动力学改变及相关因素影响。方法对老年ＬＡ患者（Ⅰ组）进行ＴＣＤ及血液流变学检查，结果与老年缺血性脑血管病患者（Ⅱ组）及对照组（Ⅲ组）比较，并对相关因素进行分析。结果Ⅰ组、Ⅱ组血液粘滞性增高，脑血流量减少，与Ⅲ组比较差异显著。Ⅰ组脑血流量下降更明显。发病与老龄、高血压、长期吸烟及多发性脑梗塞关系密切。结论缺血、缺氧、低灌流是脑白质病变的主要原因。血液的高粘状态可能是诱发因素之一。

Objective To find out cerebral hemodynamic changes and the influence of correlation factor in patients with senile leukoaraiosis (LA). Methods Examining separately LA patients(group I),senile ischemic cerebrovascular disease patients(group Ⅱ)and the control old people(group Ⅲ)with Transcranial Doppler(TCD)and hemorrheology method.The correlation factors were analyzed. Results In group Ⅰ and group Ⅱ,the blood viscosity was higher,but the cerebral blood flow was lower than those of the group Ⅲ.In particular,the cerebral blood flow of the group I was the lowest.The pathogenesis was closely related to senility,hypertension,long term smoking and multiple cerebral infarction. Conclusions The primary reason of LA is cerebral ischemia and hypoxia.Hyperviscosity of blood may be one of the inducing factors.
