帕罗西汀与阿米替林对烧伤后情绪的疗效比较

Comparison on the efficacy of paroxetine and amitriptyline in acute burnt patients with psychological disorders

目的:研究帕罗西汀与阿米替林对烧伤后情绪的临床疗效和安全性。方法:将57例伴有情绪障碍的烧伤患者随机分为两组,治疗组用帕罗西汀,对照组用阿米替林改善情绪障碍。治疗8周后行汉密尔顿抑郁/焦虑量表(HAMD/HAMA)评分,比较两组的治疗效果;并观察不良事件和副作用的发生。结果:治疗组与对照组治疗前后的HAMD/HAMA评分有显著性差异(P<0.05)。两组治疗后的HAMD/HAMA评分和显效率无显著性差异(P>0.05),但治疗组患者出现不良反应较对照组小。结论:帕罗西汀治疗烧伤后情绪障碍是安全和有效的,阿米替林也有很好的疗效,但副作用较大。

Objective：To investigate the efficacy and safety of paroxetine and amitriptyline on acute burn patients with psychological disorders. Methods： Fifty and seven patients suffered from acute burnt accompanied with depression disorders and/or anxiety disorders were randomly divided in to two groups treated with paroxetine and amitriptyline, respectively. After 8 weeks, the scores of HAMD/HAMA scale of two groups were compared on the treatment efficacy. Results：The scores of HAMD/HAMA scale were significantly different between two groups before and after treatment （P〈0.05） ,while the scores of HAMD/HAMA scale and the effective ratio were not significantly different between two groups after treatment （P〉0. 05） . The adverse effects occurred in tested group was slighter than that in the control group. Conclusion：Paroxetine is effective and safe and should be considered as first-line treatment in burnt patients with depression/anxiety. Amitriptyline also has a good clinical response but the severe complication make it as second-line antidepressant.
